How to fill out the form
- Team Name: only accepts characters (a to z), numbers (0 to 9)and the following special characters +,-#._ and space of course.
- E-mail Address: must be a valid email address as this will be your main method of communication with the game.
- Repeat E-mail Address: must be identical to your email address. Used to make sure there is no typing error in the address.
- Password: this is used for verification purposes only. You should NOT use any secure password which you use for secure sites. Choose a word or phrase you can remember which is unique to you.
- Repeat Password: must be identical to your password in every respect.
- Weekly Reminder: if you select the "Yes" button you will receive an email reminder to change your team every Thursday where there are Premier League matches in the following week. This reminder will be sent to the email address you entered above.
- Terms and Conditions: You must agree to the terms and conditions of the Golden Boot Game by clicking on the box before you submit the registration form.
